Health Equity - Health on the Go!

The COVID-19 pandemic has struck Black, Indigenous and People of Color (BIPOC) communities in greater percentages compared to other populations. Both Brooklyn Park and Brooklyn Center have high concentrations of those who identify as BIPOC. Additionally, access to health care, transportation costs, language barriers and trust in government all impact BIPOC communities to a higher degree than white communities.  

To address health inequity we have launched, and have recently completed the Health on the Go! pilot project. 

Pilot Project Description

In December 2020, the Cities of Brooklyn Center and Brooklyn Park, community-based organizations, Hennepin County and the Brooklyn Bridge Alliance for Youth launched a 1-year pilot project with two priorities: 1) To deploy health resources to those most impacted by COVID-19 and 2) Bring together the health assets and resources in the Cities of Brooklyn Center and Brooklyn Park to eliminate health disparities and advance health equity overall.

This pilot project was completed in August 2021. The final report on results and a presentation to the Cities can be found here. Each city has received these results and is exploring their next steps in addressing health equity. Pilot Project Results

Data collected indicates that residents’ have a positive experience with Health on the Go! (HOTG) and they need more resources to support their well-being beyond COVID-19 vaccinations and testing. With the appropriate funding support, transitioning from pilot to a full program can be a success. During transition planning we have confirmed that ALL partners have expanded resources to work collaboratively on HOTG, and that there is a commitment between all partners to continue building the capacities of community to address health equity for their diverse, increasingly mixed, and historically underserved demographics.
